ディズニーホテル 未成年の方のみでのご宿泊について
ご宿泊者が未成年の方で1名様または未成年の方同士の場合、法定代理人（親権者等）に「同意書」の提出をお願いしております。
また、宿泊時点で15才未満または中学生以下の方のご宿泊には、法定代理人または法定代理人が同意する保護者の同行が必要です。
ご宿泊開始日の7日前までに、FAXまたは郵送にて同意書のご送付をお願いいたします。

また、東京ディズニーシー・ファンタジースプリングスホテル、ディズニーアンバサダーホテル、東京ディズニーシー・ホテルミラコスタ、東京ディズニーランドホテル、東京ディズニーリゾート・トイ・ストーリーホテルのうち、複数のホテルに宿泊される場合は、宿泊されるすべてのホテルにチェックし、日付をご記入いただいたうえ、1泊目のホテルへご送付をお願いいたします。

- 東京ディズニーセレブレーションホテルの同意書とは別書式になりますので、ご注意ください。
- 東京ディズニーリゾート・バケーションパッケージをご利用の方は、東京ディズニーリゾート・バケーションパッケージ専用の同意書をご利用ください。
- 旅行会社にて宿泊予約をされた場合は、旅行会社指定の未成年同意書を旅行会社に提出された方は、当書面のホテルへの提出は不要です。
If a minor or a group of minors will be staying at a Disney Hotel, they are required to submit a form of consent filled out by a parent or other person with parental authority. Guests of 15 or younger at the time of stay must be accompanied by a legal representative, or a legal guardian who has the consent of the legal representative, to stay at the hotel. The form of consent must be submitted, either by fax or by postal mail, at least seven days before the check-in day.

If the Guest(s) will be staying at two or all three of the Disney Hotels (Tokyo DisneySea Fantasy Springs Hotel, Disney Ambassador Hotel, Tokyo DisneySea Hotel MiraCosta, Tokyo Disneyland Hotel, or Tokyo Disney Resort Toy Story Hotel), check the boxes next to all of the hotels where they will be staying, fill in the date of check-in below, and submit the form to the first hotel where they will stay.
- One form of consent must be submitted for each Guest of minor age.
- A separate form of consent is required for Guests staying at Tokyo Disney Celebration Hotel.
- Guests booked with Tokyo Disney Resort Vacation Packages are required to use the form of consent specified by Vacation Packages.
- Regarding stays during the dates shown above, guests who have made a reservation through a travel agency do not need to submit the consent form for minors submitted to the travel agency to the hotel.
